To request access to your personal health information (medical records), you must complete the Request to Access Personal Health Information form in full and submit it by email, fax, or mail:

The Patient Relations Office holds a neutral position in the hospital. Their role is to ensure that concerns are heard and that patient concerns receive attention and follow up. If you have questions or concerns about your care, please talk to the nurse, doctor, or any member of your care team.
If a problem arises that can’t be resolved with your care providers, you or a family member can request that the Patient Relations Officer become involved. Speaking up will improve your experience at HSC; it will not compromise your care.
